A one-vehicle accident Friday, May 30, left one Grayson County resident dead and three children injured. According to a report from state trooper David Norris, a truck driven by Paula Hazelwood was eastbound on Millerstown Road when the right rear tire tread failed.

Hazelwood lost control of the vehicle and the truck left the road on the left shoulder, ran through a ditch, sheared a tree and flipped several times before coming to rest upright.

She was ejected from the vehicle and later pronounced dead at Twin Lakes Regional Medical Center.

Three children, which were also in the vehicle, were transported to Twin Lakes by Grayson County EMS. Two of the children were then transported by StatCare to Kosair's Children's Hospital.

The youngest child was admitted at Twin Lakes Regional Medical Center for 24-hour observation, according to a hospital spokesperson.

The accident is under investigation by Trooper Norris with the Kentucky State Police. Clarkson Fire Department, Grayson County EMS and the Grayson County Coroner responded to the accident.
