She was born June 22, 1928 in Grayson County, the daughter of Van and Era Higdon Cook.
She was preceded in death by her first husband, Owen Cook; and her second husband Robert Hack; one son, Willard Cook; and one daughter, Faye King.
She is survived by one son, Brad Cook of Caneyville, Ky.; one daughter, Barbara Embry of Leitchfield; one sister, Corene Woodcock of Caneyville; one brother, Floyd Cook of Caneyville; six grandchildren, and six great-grandchildren.
Funeral services were held Saturday, May 6, 2006 at Watson & Hunt Funeral Chapel in Leitchfield.
Burial was in the Clarkson Cemetery.
